Records to Bring to Your Initial Consultation
Enabling Us to Better Assist You
Getting Started: Gather relevant documentation
* Compile all records related to your child's educational needs so that you can establish a strong case.
* Requests for records from your child's school or school district office must be made in writing. It is essential to obtain proof of delivery of your written request. Ask the recipient of your written request to sign and date the document and provide you a photocopy for your records.
Bring a copy (that you do not need returned to you) of all of the following items to your first appointment:
* All correspondence, including e-mails, between you and your child's school or school district.
* Any records from private professionals who have provided your child with medical, physiological, educational and related services, that may be relevant to your child receiving needed educational supports and services.
* Your child's cumulative school records, which should include the 504 plan if your child has one.
* If your child already has an IEP, you should request the Special Education records, including but not limited to all IEPs and district educational assessments.
* If your child has missed a great deal of school, ask for attendance records.
* If problematic behaviors have occurred at school, ask for your child's disciplinary records.
